## Why does the joint keep getting sore?

After years of use, arthritis or other wear may make a joint stiff. An old injury can keep bothering the same area long after it healed. In a shoulder, an overworked tendon often hurts during lifting or reaching.

A worn joint may ache after walking and then ease with rest. Stiffness may be worse after sitting through a meal. A clinician uses the timing and sore area to tell slow wear from a new injury.

## What can make the soreness easier?

At 12 months the change in knee pain was -2.1 vs -1.8 points (difference -0.4; 95% CI -0.9 to 0.2; P=.17) and the change in medial tibial cartilage volume was -1.4% vs -1.2% (difference -0.2%; P=.81). Twenty-nine of 31 prespecified secondary outcomes showed no between-group difference. The authors concluded the findings do not support the use of PRP for knee OA.

2. Blood from seven donors was processed through five commercial PRP systems (Arthrex Angel, Emcyte Genesis CS, Arteriocyte Magellan, Harvest SmartPrep, Biomet GPS III). White-cell concentration ranged from 11.0 to 27.3 thousand/uL between systems, neutrophil concentration from 0.6 to 9.4 thousand/uL, red-cell concentration from under 1.1 to 3.2 million/uL, and pH from 6.95 to over 7.26. The authors advise caution in interpreting clinical PRP results because the product itself differs by device.

A meta-analysis of 34 RCTs (1,403 PRP knees vs 1,426 controls) found WOMAC favoured PRP over placebo at 12 months and over hyaluronic acid at 6 and 12 months, and favoured PRP over steroids on VAS pain, KOOS pain, daily function and quality of life at 6 months. Crucially, the authors state that the superiority of PRP DID NOT REACH the minimal clinically important difference for any outcome and the quality of evidence was low.

The AAOS patient-education FAQ on orthobiologics states that because orthobiologics are relatively new the evidence supporting their use is LIMITED, that rigorous testing of effectiveness in most orthopedic conditions is lacking, and that preliminary results are encouraging but hard to evaluate. It names tendinopathies such as tennis elbow, pain from early knee osteoarthritis, adjunct healing after rotator cuff repair, and avascular necrosis as the settings where biologic therapies have shown promise, and notes that stem cell treatments not derived from the patient's own body and further manipulated in a laboratory can only be offered inside an FDA-approved clinical trial.
